Completed forms can be turned in during lunch time in the Commons on the 1st Monday of the month starting October 2013 until April 2014. Collection dates are Oct 7, Nov 4, Dec 2, Jan 6, Feb 3, Mar 3, and April 14 which will be the FINAL turn in date for Seniors.
